This form is for retreat participants who will commute to the camp site daily. Every adult commuter with children must complete a retreat registration form and submit a Shocco waiver form. Activity Waiver: In consideration of my child being allowed to participate in activities at the Shocco Springs Children’s Camp from December 26 to December 28, 2025, sponsored by Birmingham Chinese Christian Church and Huntsville Chinese Christian Church, I hereby release, discharge, and agree to hold harmless both churches, including their ministers, officers, employees, agents, and all volunteer personnel from any liability, claims, demands, and causes of action arising from any loss, damage, injury, or illness sustained by my child during the camp. I also agree to indemnify both churches and their representatives from any claims, damages, and liabilities (including reasonable attorney fees, litigation expenses, and court costs) that are required to pay arising from any injury or damage during the camp activities.
Medical Waiver: In the event of a medical emergency, I authorize the staff and volunteers of Birmingham Chinese Christian Church and Huntsville Chinese Christian Church to arrange for medical treatment deemed necessary for my child's well-being. I release both churches and their representatives from any liability related to medical care provided. I accept all financial responsibilities for medical expenses incurred during emergencies and understand that efforts will be made to contact me in the event of a serious illness or injury.
